Passport Services
The American Citizen Services (ACS) Unit of the Consulate provides passport services to U.S. citizens by issuing first time passport, replacing lost/stolen/mutilated passports and renewing old passports.
All applicants irrespective of their age have to apply in person except applicants who are eligible to use DS-82: Application for passport by mails-renewals only (See form below). Minor passport applicants below the age of 16 must be accompanied by both their parents. In case one of the parents are unable to appear in person, they should complete DS-3053 form giving statement of consent for issuance of minor passport.Children aged 16 or above must sign their own passport application.Children aged 16 or above must sign their own passport application.
Renewal / Replacement: If the applicant meets the following requirements, the application is considered to be for passport renewal :
Applicant's most recent U.S. passport
- was issued in his/her name after his/her 16 th birthday
- was issued within the last fifteen years
- has not been expired for more than two years.
Applicant is able to submit his/her most recent U.S. passport.
Otherwise, the application is considered to be for passport replacement. There are different formats and fees for passport replacement and passport renewal.
Requirements for a new passport / replacement of an old one:

DS-11 (Application for Passport/Registration). See forms below.
Evidence of U.S. citizenship:
Evidence of Identity : This is required if the only evidence of citizenship provided by the applicant is item 2.d or 2.e above in which case the applicant needs to support it with evidence of identity, such as a driver's license, or other official photo identification.
If a passport applicant is claiming derivative citizenship through birth abroad to American citizen parent(s), then evidence that the applicant acquired U.S. citizenship at birth under the law in effect at the time of the applicant's birth is also required. More information is available under

For lost/stolen/mutilated passports : A police report explaining how your passport was lost or stolen along with affidavit of lost passport and statement of loss/theft of passport. These are required in addition to the documents mentioned in 1-6 above.
The fee for first time issuance of an adult(over 16 years of age), 10-year passport is US$ 100(Rs.4,800) and that of a minor passport (under the age of 16) is US$85(Rs.4,080).
The fee for replacing lost/stolen adult passport is US$100 (Rs.4,800) and for a minor is US$ 85 (Rs.4,080).
The fee for an adult (over 16 years of age) passport(10-year) renewal is US$75(Rs.3,600).
Requirements for Passport Renewals:
- Completed Form DS-82 (Application for Passport by Mail)for adults and Form DS-11 (Application for Passport/Registration) for minors.(See forms below).
- Photo instructions for U.S. Passports.
- The applicant's most recent passport, issued since he/she became 16 years old and within the last 12 years.
- Social Security number, if issued.
- For the renewal fee of a passport for both adults and minors (see no. 8 above)
- U.S. Passport Issuance for children under 16 years of age.
In addition to the above, the following criteria will be applicable for minors for passport renewals :
- The applicant (minor) is physically present in the consular district of Kolkata (Evidence may be requested to prove this).
- The minor and, if applicable, the American citizen parent(s) or guardian are registered with the American Consulate General in Kolkata.
- There is no child custody aspect to the case.
- The applicant's most recent passport was issued within the past 12 years, by which the applicant (minor) can be recognized and identified (the minor's identity must be clearly established in each case).
- If the child cannot be identified through comparison with the photo in the previous passport, other proof of identity such as school records with photo identification or a progression of family photographs taken during the intervening years.
NOTE: All applicants with Form DS-11 (Application for Passport/Registration) must apply in person at the Consular Section.

Changing the name in a U.S. Passport: In order to change the name in the passport, the U.S. passport holder should bring the passport and proof of legal change of name to the ACS unit of the Consulate. Proof of legal change of name can include an original or certified copy of a court order, a marriage decree, or a divorce decree stipulating the restoration of maiden name. Please complete form as appropriate.The links are given below.
Adding visa pages to a U.S. Passport: Blank visa pages can be added to U.S. passports immediately,after submitting form DS-4085, Click to download and getting a namecheck clearance, free of charge, upon request. U.S. passport holders do not have to come in person to request this service and may send someone else with the passport. No authorization letter is required.
